Care Manager MSW Per Diem/Day

We are seeking a skilled Care Manager MSW to join our team at St. Joseph Hospital of Orange. As a Care Manager, you will play a vital role in providing exceptional patient care and support.
Key Responsibilities- Collaborate with patients, nurses, social workers, physicians, and other healthcare professionals to develop and implement comprehensive care plans.
- Provide emotional support and guidance to patients and their families, addressing their psychosocial needs and concerns.
- Facilitate communication and coordination among healthcare providers to ensure seamless care transitions and optimal patient outcomes.
- Conduct thorough assessments and evaluations to identify patients' needs and develop targeted interventions.
- Develop and maintain relationships with community resources and services to ensure patients receive the support they need.
- Master's Degree in Social Work, Psychology, or Counseling, or equivalent education/experience.
- 6 months of clinical experience in a healthcare setting, preferably in a hospital or medical office/clinic.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to work independently and as part of a team, with a focus on delivering high-quality patient care.
- Certification in Case Management (CCM, ACM) or LCSW.
- 3 years of experience in utilization management or case management.
- Experience with psychiatric chemical dependency treatment and managed care.
