Job Description

The Esperanza Center Health Clinic is seeking a highly skilled Family Nurse Practitioner to join our team. As a key member of our healthcare team, you will provide primary care to patients of all ages, working collaboratively with medical and program staff to deliver exceptional patient outcomes.


Key Responsibilities
  1. Patient Care: Provide high-quality primary care to patients, including routine check-ups, health screenings, and treatment of acute and chronic conditions.
  2. Program Development: Collaborate with medical and program staff to develop and implement effective preventative health programs.
  3. Patient Assessment: Conduct thorough assessments of patients' needs, identifying opportunities for referral to social services and other support resources.
  4. Record Keeping: Maintain accurate and up-to-date patient clinical records, ensuring compliance with regulatory requirements.
  5. Quality Improvement: Participate in quality assurance and peer review activities to continuously improve patient care and outcomes.

Qualifications
  • Master's degree in Nursing and Certified Registered Family Nurse Practitioner (FNP) license with the Maryland Board of Nursing.
  • Experience in primary care, chronic disease management, and minor procedures.
  • Strong communication and interpersonal skills, with ability to work effectively with diverse patient populations.
  • Bilingual proficiency in Spanish and English.
  • Familiarity with electronic medical record systems and Microsoft Office applications.
