Overview

At St. Luke's Health System, our team of skilled Home Health RNs is dedicated to delivering exceptional patient experiences in the comfort of their own homes. We strive to build a positive, supportive, and inclusive culture that prioritizes the well-being of our patients and their families.

Our home health team provides comprehensive care to patients of all ages and stages of life, from newborns to seniors. We offer the latest home health techniques and technology to ensure that our patients receive the highest quality care possible. Our team works closely with St. Luke's Hospice team to provide seamless transitions and comprehensive care.

Key Responsibilities:

  • Provide comprehensive care to patients with new diagnoses, medications, or recent discharges from the ER
  • Assess physical and psychological needs to improve quality of life
  • Coordinate resources and educate patients and their families upon discharge

What You Can Expect:

  • Part-time schedule with three days a week (8:30a-5:00p) with rotating evening call and weekends
  • Manageable case-loads with daily visit requirements below state and national averages
  • Pay for drive time and mileage reimbursement
  • Differential pay for nights and weekends
  • Preceptor training available
  • Sign-on bonus available for new hires

Qualifications:

  • Education: ASN or BSN Degree
  • Experience: 1 year of RN experience; Home Health or Acute Care experience helpful
  • Licenses/Certifications: Current RN License in the State of Practice; current Basic Life Support (BLS) Certification
  • Valid Driver's License and proof of Auto Insurance
