Job Title: Travel Nurse RN Case Manager

Tact Staff is seeking a skilled Travel Nurse RN Case Manager to join our team in Gainesville, Florida.

Job Summary:

We are looking for a highly motivated and experienced RN Case Manager to work in our Acute Care Case Management department. As a Travel Nurse RN Case Manager, you will be responsible for providing high-quality patient care and managing patient flow in a fast-paced hospital setting.

Responsibilities:
  • Provide direct patient care and manage patient flow in the Acute Care Case Management department.
  • Assess patient needs and develop individualized care plans to ensure optimal patient outcomes.
  • Collaborate with interdisciplinary teams to coordinate patient care and services.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
  • Manage patient records and documentation to ensure accuracy and compliance with regulatory requirements.
Requirements:
  • Current RN licensure in the state of Florida.
  • Minimum 2 years of experience in Acute Care Case Management or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize multiple tasks.
  • Basic computer skills and proficiency in electronic medical records.
