Salary: $63.00 - $115.00 per visit

Job Description: As a Home Health Physical Therapist, you will provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to an illness or injury. You will work as part of a home health team focused on the patient's plan of care and goals.

  • Observe movements, conduct assessments, document progress and modify care plan as needed
  • Assist patient with exercises and hands-on therapy to manage pain and increase mobility
  • Educate patients on their plan of care, established goals, exercises, and self-care

Required Skills and Qualifications: To succeed as a Home Health Physical Therapist, you will need to possess strong clinical judgment, interpersonal skills, dexterity, and compassion. A graduate of an accredited Physical Therapist Program and active PT license in FL are required. Minimum of 1 year of physical therapy experience in home healthcare and CPR certification are also necessary.
