Sigma HomeCare is seeking a compassionate and dedicated Direct Care Worker to provide exceptional care to our clients with severe mental illness, serious emotional disturbance, intellectual and developmental disabilities, and those in need of mentorship programs.
As a Direct Care Worker, you will play a vital role in improving the quality of life for our clients. You will be responsible for providing comprehensive community support services and respite, ensuring that our clients receive the highest standard of care in the comfort of their homes.
Key Responsibilities- Provide direct care and support to clients in their homes, assisting with daily living activities such as bathing, dressing, grooming, and meal preparation.
- Monitor clients' medications as prescribed by healthcare professionals, observing for any side effects or adverse reactions and reporting any concerns to the appropriate healthcare provider.
- Implement and follow individualized care plans tailored to each client's specific needs, regularly reviewing and updating care plans in collaboration with healthcare professionals and family members.
- Monitor clients' progress and behavior, keeping detailed records of their daily activities, health status, and any significant changes, maintaining accurate and up-to-date documentation for each client.
- Communicate effectively with clients, their families, and other healthcare professionals to ensure coordinated and comprehensive care, providing updates on clients' progress and addressing any concerns or questions.
- Transport clients to appointments, community activities, and other necessary outings, ensuring clients' safety and comfort during transportation.
- Maintain a clean and safe environment for clients, assisting with light housekeeping duties, including laundry, cleaning, and organizing clients' living spaces.
- High school diploma or equivalent.
- Strong communication and interpersonal skills, with the ability to build positive relationships with clients and their families.
- Ability to work independently and as part of a team, demonstrating reliability and accountability.
- Valid driver's license and reliable transportation are required.
- Must pass a background check and drug screening.
- Must have a smartphone to access work-related applications and communicate with the team.
