We are seeking a highly skilled Pharmacist to join our team at PharMerica, a leading provider of pharmacy services to long-term care facilities. As a Pharmacist in this role, you will be responsible for ensuring the safe and effective use of medications in our facilities.
Key Responsibilities- Evaluate physician medication orders for appropriateness, making recommendations for therapy changes as needed.
- Maintain accurate and up-to-date patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medications.
- Ensure that appropriate laboratory tests are ordered, evaluated, and discussed with the healthcare team.
- Collaborate with the healthcare team to provide clinical and technical support as needed.
- Document all communication with physicians, nurses, and other members of the healthcare team in the medical record.
- Implement policies and procedures for the safe procurement, storage, distribution, and use of medications and biologicals.
- Obtain and maintain all required documentation in accordance with federal and state regulatory agencies and policy.
- Inform the Pharmacy Manager of medication-related errors.
- Assist facility staff with procedures related to safe and effective methods of procuring, maintaining, administering, and disposing of medications.
- Bachelor's Degree in Pharmacy; Pharm D preferred.
- One year experience in long-term care.
- Licenses/Certifications: As required by government regulations and business needs.
- Detail-oriented, goal-oriented, team player, and excellent oral communication skills.
