Job Summary

Key Responsibilities
  • Evaluate and address the needs of residents, ensuring their care plans are met and exceeded.
  • Orient new residents to our facility and participate in care planning meetings to ensure a smooth transition.
  • Identify and resolve resident adjustment issues, promoting a positive living environment.
  • Assist residents in maintaining family and community connections, as well as establishing new ones.
  • Encourage resident participation in community activities and identify needs for services and entitlements.
  • Coordinate the work of case management and service providers to ensure seamless care.
  • Assist residents in need of alternative living arrangements, making and executing sound discharge or transfer plans.
  • Oversee and document case management needs and activities, including plan of care, aide care plans, and medication assistance.
  • Work within the confines of NYS DOH regulations and Resident Rights, maintaining HIPAA confidentiality.
  • Orient and supervise Charge Nurses and aides, ensuring required staffing schedules are met.
  • Support the Wellness Program and Fall Prevention under the direction of the Wellness Director/ Director of Resident Services.
  • Conduct meetings with families and/or responsible parties to foster good communication, exemplary resident care, and great customer service.
  • Attend marketing events as required.
  • Assist in maintaining on-call schedules and on-call responsibilities, ensuring availability for consultation to the nursing staff.
  • Assist with the interview, hire, discipline, termination, and supervision of nursing staff through participation in staff meetings, orientation, and required in-services.
  • Follow Viva Senior Living's Policies & Procedures.
  • Assist with QA activities.
  • Assist in maintaining all Viva Senior Living's required reporting logs.
  • Assist in maintaining the department budget, as set forth by Viva Senior Living.
Requirements
  • Annual health requirements: Must provide annual verification of a negative TB skin test and proof of full COVID-19 vaccination with exceptions for medical and religious accommodations.
  • Physical and sensory requirements: Must be able to move intermittently throughout the workday, cope with mental and emotional stress, see and hear or use prosthetics to function adequately, and meet general health requirements set forth by the policies of this facility.
  • Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, agitated residents.
  • Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
  • May be necessary to assist in the evacuation of residents during emergency situations.
