We are seeking a compassionate and motivated Physical Therapist to join our winning team at JAG Physical Therapy, a comprehensive outpatient orthopedic physical therapy company with 100 facilities throughout Pennsylvania, New Jersey, and New York.
Job SummaryThe Physical Therapist will be responsible for assessing patients and creating customized treatment plans to aid in their recovery and functional independence. This role requires maintaining patient confidentiality, excellent clinical decision-making skills, and collaborative work with both patients and healthcare teams.
The ideal candidate will have a strong understanding of musculoskeletal interventions, including therapeutic exercise, joint mobilization, and modalities. They will also have experience with EMR software and be able to collect and record patient care data in accordance with department guidelines.

Responsibilities- Collects and records patient care data that follows format and guidelines of the patient care service, regulatory agencies, and third-party payers which reflects an understanding of reimbursement systems and their impact on patient care.
- Additionally records daily billing and follows department guidelines for productivity.
- Prepares for the evaluation in accordance with department policies/procedures and standards of practice, including obtaining patient history, exhibiting knowledge of patient diagnoses, and researching unfamiliar diagnoses.
- Selects and administers tests and measures.
- Synthesizes evaluation results to formulate the diagnosis, goals, prognosis, and plan of care.
- Establishes treatment plan based on evaluation results, goals, and best practice. Collaborates with interdisciplinary team, patients, and patient's support system.
- Assesses and re-assesses need for modifications to plan of care and goals.
- Identifies need for consultations and/or refers the patient to another healthcare professional or healthcare services. Initiates and completes discharge plan.
- Provides education/training to patient and/or patient support system.
The estimated annual salary for this position is $85,000 - $110,000, depending on experience.
