Job Summary:We are looking for a highly motivated and experienced Physical Therapist to work in a Skilled Nursing Facility (SNF) / Long-Term Care (LTC) setting. The ideal candidate will have a strong background in physical therapy and excellent communication skills.
Responsibilities:- Provide physical therapy services to patients in a SNF / LTC setting
- Develop and implement individualized treatment plans to meet patient needs
- Conduct thorough assessments and evaluations to identify patient goals and objectives
- Collaborate with interdisciplinary teams to ensure comprehensive patient care
- Document patient progress and maintain accurate records
- 1 year of recent experience as a Physical Therapist in a SNF / LTC setting
