Mass General Brigham is seeking a skilled Social Work Specialist to support patients and their families in navigating the complexities of illness, personal issues, and emotional/mental health challenges. The ideal candidate will possess a Master's degree in Social Work and be licensed in MA at the Licensed Independent Social Work level or licensed clinical Social Work level.

Key Responsibilities
  • Assist patients and their families in coping with stress and difficulties related to illness, personal issues, and financial difficulties.
  • Collaborate with nurse care managers and other multidisciplinary team members to develop safe and effective discharge plans.
  • Support the mission of Mass General Brigham and participate in Performance Improvement activities.

The successful candidate will have experience working in an acute care setting and possess strong assessment and planning skills, as well as the ability to communicate effectively verbally and in writing.
