Responsibilities
  • Assess and Plan Patient Care: Utilize the nursing process to assess patients' needs, develop individualized care plans, and implement evidence-based interventions to achieve optimal patient outcomes.
  • Provide Direct Patient Care: Administer medications, perform wound care, and provide other nursing interventions as needed to promote patients' physical and emotional well-being.
  • Collaborate with Healthcare Team: Work closely with physicians, therapists, and other healthcare professionals to ensure seamless care coordination and effective communication.
  • Document Patient Information: Accurately and thoroughly document patient information, including medical history, treatment plans, and progress notes, to maintain accurate and up-to-date records.
  • Conduct Home Visits: Visit patients in their homes to provide care, answer questions, and address concerns in a timely and compassionate manner.
Requirements
  • Current RN License: Possess a current and active Registered Nurse license in the state where you will be practicing.
  • Health Certificate: Maintain a current health certificate, as required by facility contract or state regulation.
  • PPD or Chest X-Ray: Provide proof of a current PPD or chest X-ray, as required by facility contract or state regulation.
  • BLS Certification: Hold a current BLS (Basic Life Support) certification.
  • Experience: One year of prior RN experience is preferred, but not required.
