Trumbull Connecticut, United States Yale New Haven Health Full time

Overview:

At Yale New Haven Health, we believe that every team member should embrace our Vision, support our Mission, and embody our Values. Our core values - integrity, patient-centered care, respect, accountability, and compassion - are essential in guiding our actions as professionals every day.

The Physician Assistant or Nurse Practitioner is a licensed healthcare provider who collaborates with an attending physician across various practice environments within our organization.

We are committed to delivering exceptional diagnostic and therapeutic patient care, adhering to the highest standards as part of a multidisciplinary team.

Our focus is on creating a patient-centered atmosphere that addresses the comprehensive needs of the individual, including physical, psychosocial, spiritual, and functional aspects for patients, families, and significant others.

This role serves as a key professional in ensuring continuity of care, addressing and resolving issues that may arise within the scope of practice standards and specialty-specific guidelines.

EEO/AA/Disability/Veteran

Responsibilities:

  • Clinical Care: The Physician Assistant (PA) or Nurse Practitioner (NP) conducts assessments, triages, and manages therapeutic interventions for patient and family needs throughout the healthcare journey as a vital member of an interdisciplinary team. Whether in inpatient, ambulatory, or outpatient settings, the NP/PA ensures optimal patient care, from diagnosis to prognosis. This individual collaborates with supervising physicians in accordance with state laws and hospital regulations, hospital policies, protocols, and clinical pathways, and may provide autonomous team-based care. Responsibilities include obtaining medical histories, performing physical examinations, reviewing and ordering diagnostic tests, and formulating and managing treatment plans. The NP/PA may perform procedures, prescribe treatments and medications in compliance with institutional and state guidelines, and may have autonomous practice in certain specialties. Additionally, the NP/PA may assist in operative cases as long as they meet position scope and educational prerequisites.
  • Patient Education & Psychosocial Support: The NP/PA educates patients and families about care and available resources. In collaboration with the supervising physician, this includes providing admission instructions, treatment plans, discussing potential side effects of treatments and medications, and discharge planning. The NP/PA also offers psychosocial support to patients and their families in a compassionate and professional manner.
  • Information Management: The NP/PA ensures accurate documentation in accordance with hospital and regulatory requirements, safeguarding all forms of information from unauthorized access or disclosure.
  • Quality Management: The NP/PA is expected to uphold high standards of ethical behavior, professionalism, compassion, and dedication to patient care, ensuring compliance with healthcare regulatory requirements and standards.
  • Professional Development: The NP/PA actively participates in projects that enhance and maintain professional competency. They may serve as a clinical resource for new staff and provide orientation pertinent to clinical education. The NP/PA consistently demonstrates reliability by reporting safety events and near misses, and engages in projects that advance their professional skills. They maintain all employee requirements set by hospital policy and may mentor NP/PA students and other healthcare professionals. Collaboration on research, quality assurance, and performance improvement projects is also encouraged.

Qualifications:

EDUCATION:
Graduation from an accredited Nurse Practitioner or Physician Assistant Program.

EXPERIENCE:
Clinical experience as a NP/PA in the relevant specialty/service line is preferred but not mandatory.

APRNs, PAs, CRNAs, and CNMs must be appointed to the Affiliated Medical Staff of a Yale New Haven Health Hospital and credentialed through the Medical Staff process.

Evidence of current competence to practice as an APRN, PA, CRNA, or CNM must be obtainable during the Medical Staff appointment process.

LICENSURE:

Active PA Certification by the National Commission of Certification of Physician Assistants (NCCPA), Connecticut State Physician Assistant License, ACLS/BLS Certification as required for the position, DEA License as needed for pediatric roles, PALS for neonatal care, and NRP Certification.

Additional licensure, certifications, and/or continuing education credits may be required during employment based on position and specialty area.

Additional Information:

A minimum of 2 years of experience is preferred, and PEARS/PALS certification is required. The role operates Monday through Friday during daytime hours.
