Gentiva Health Services is a leading provider of hospice care, dedicated to delivering compassionate and personalized support to patients and their families. We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team as a Per Diem Hospice LPN.

Key Responsibilities

  • Provide direct patient care services, including administering medications and wound care.
  • Evaluate patient/family needs and develop individualized plans of care.
  • Communicate effectively with patients, families, and healthcare teams.
  • Prioritize tasks and manage time effectively to meet deadlines.

Requirements

  • Current LPN license in good standing.
  • Experience in hospice, home health, ICU, geriatric, or oncology settings preferred.
  • Ability to work autonomously and collaboratively as part of a team.
  • Familiarity with electronic medical records (EMRs) and other healthcare technology systems.
