Job SummaryThe successful candidate will provide physical therapy and wellness services for older adults under the supervision of a PT as part of a multidisciplinary team. This role will also involve supporting care coordination efforts by conducting regular wellness checkpoint visits with patients, communicating with patients' providers and caregivers, and assisting patients with attending telehealth visits.
