Job Overview

We are seeking an experienced RN Case Manager to join our homecare team in Pueblo, Colorado. As a pivotal member of our interdisciplinary team, you will play a crucial role in identifying the physical, psychological, social, and spiritual needs of our patients and their families.

Responsibilities
  • Initiate appropriate interventions and support for patients and families upon admission.
  • Work closely with the inter disciplinary team to provide a continuous, comprehensive, and responsive plan of care for patients.
Requirements
  • A valid non-restricted nursing license in the state of Colorado is required.
  • Must have a valid non-restricted driver's license.
  • Flexibility and availability to work a weekend schedule if needed.
  • At least 1 year of hospice or home health experience preferred.
What We Offer
  • Competitive Salary: $60,000 - $80,000 per annum, depending on experience.
  • Mileage reimbursement of sixty-two and a half cents per mile.
