Responsibilities- Assess and evaluate patient care needs, and implement individualized care plans in collaboration with the healthcare team.
- Administer medications, perform wound care, and monitor vital signs as needed.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Participate in performance improvement activities to enhance patient outcomes and satisfaction.
- Collaborate with multidisciplinary teams to develop and implement care plans that meet the unique needs of each patient.
- Registered Nurse (RN) licensure and current certification in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS).
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) degree.
- 1 year of experience in a similar role, preferably in a hospital setting.
