Home Infusion Registered Nurse

The Home Infusion Registered Nurse - PRN is a key member of our team, responsible for delivering high-quality patient care in the ambulatory infusion suite and home setting. As a skilled nurse, you will coordinate and administer intravenous (IV) treatment infusions, teach patients and caregivers about home IV management, and work collaboratively with the pharmacy team to establish priorities and goals of patient care.
Responsibilities- Provide professional nursing care to assigned patients in need of IV treatment.
- Administer IV therapeutic treatments at the ambulatory infusion suite (AIS) and/or patient's home.
- Determine appropriate venipuncture and/or catheter site.
- Observe patients' IV treatment and record observations in patient medical records.
- Provide information to patients and caregivers regarding intravenous treatment and procedures.
- Provide infusion and other skilled nursing care to patients.
- Complete patient assessment and reassessments and patient and caregiver teaching.
- Assist team in the development and revision of patient's plan of care; implement physician orders.
- Monitor patient response, respond to emergency patient needs, and provide psychosocial support.
- Provide pre-discharge patient assessments when appropriate.
- Complete and submit all required patient care documentation and reports accurately.
- Follow infection control guidelines while working with hazardous materials (i.e., handling of cytotoxic agents, handling of blood, and body fluids).
- Assume potential responsibility for occasional on-call phone triage after hours to address patient needs.
- Attend scheduled patient care conferences, all in-services, departmental meetings, etc., as requested.
- Associate or Bachelor's degree in Nursing required.
- Continuing education necessary to maintain current licensure where required by state or professional organization.
- Minimum of two (2) years of nursing experience in a clinical setting involving collaboration and management of patient care, patient assessment skills, and venous access skills required.
- Peripheral IV skills required.
- Pediatric experience preferred.
- PICC/PORT training desired.
- Active and unencumbered Registered Nurse license in state(s) of employment required.
- Current CPR certification required.
- INS/ONS certification desired.
The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
