As a Hospice CNA, you will play a vital role in providing high-quality care to patients in their own homes. You will work closely with our interdisciplinary team to develop and implement personalized care plans that meet the unique needs of each patient.
Responsibilities:- Provide direct patient care, including bathing, grooming, and toileting
- Assist patients with ambulation, transfers, and range of motion exercises
- Monitor and report changes in patient condition to the supervisor
- Follow the assignment sheet/service plan and perform only tasks that are assigned
- Active Nursing Assistant Certification (CNA) in Michigan
- High school diploma or equivalent
- Knowledge of state and federal healthcare laws and regulations
- Compassionate nature and good communication skills
