Job Overview
Our organization, Mental Health Association, is seeking a highly skilled Care Coordinator Specialist to join our team. As a key member of our Partial Care Program, you will be responsible for providing essential care services to individuals with severe and persistent mental illness.

About the Role
This is a dynamic opportunity for a motivated professional to work closely with staff, supervisors, and members to deliver high-quality partial care services. Your primary focus will be on implementing daily program activities, planning milieu activities, and facilitating therapy groups to promote social skills and relapse prevention.

Responsibilities
- Implement daily program activities in collaboration with staff and members
- Plan and facilitate therapy, coping, activity, and psycho-educational groups
- Collaborate with members and staff to achieve psycho-social goals and tasks
- Provide case management services, including coordination of medical, psychiatric, financial, residential, social, and legal needs
- Conduct regular goal-setting with members and provide crisis intervention as needed
- Liaise with family, boarding home staff, and local agencies serving the elderly

Requirements
- Bachelor's degree in a human services field
- At least one year of related experience
- Computer literacy and valid driver's license required
- Ability to drive a passenger van
