Nurse Practitioner

About the Role:
Monogram Health is seeking a skilled Nurse Practitioner to join our team of healthcare professionals dedicated to improving the lives of patients with chronic kidney disease and end-stage renal disease. As a Nurse Practitioner at Monogram Health, you will play a vital role in delivering high-quality, patient-centered care in the comfort of patients' homes.
Key Responsibilities:
- Conduct comprehensive annual wellness exams and provide personalized care to patients with chronic kidney disease and end-stage renal disease.
- Counsel and educate patients and families about benefits and programs available to help them live healthier lives.
- Document patient information, including medical history, medications, and treatment plans.
- Collaborate with primary care providers, specialists, and ancillary services to ensure seamless care coordination.
- Prescribe medications, order tests, and work with patients' Monogram physicians to deliver evidence-based care.
- Participate in integrated care team meetings and contribute to the development of treatment plans.
Requirements:
- Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
- Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA).
- Current and unrestricted DEA certificate.
- Ability to work autonomously and practice without direct supervision.
- Access to transportation, a valid driver's license, and car insurance.
- Proficiency with medical instruments and equipment required by the work.
- Knowledge of computer-based data management programs and information systems.
- Ability to communicate effectively with patients, families, physicians, and community representatives.
- Sound understanding of federal and state regulations, including HIPAA and OSHA.
- 2 or more years of direct patient care experience.
- Managed Care/IPA/Health Plan experience.
