Job Summary:

We are seeking a skilled Home Health Care Professional to provide direct patient care and support to our clients in the comfort of their own homes. As a member of our team, you will be responsible for assisting with daily living activities, such as personal hygiene, feeding, and mobility, while also ensuring a safe and clean environment.

Responsibilities:

* Assist patients with daily living activities, including personal hygiene, feeding, and mobility
* Provide emotional support and companionship to patients and their families
* Maintain accurate records of patient care and progress
* Collaborate with healthcare professionals to develop and implement individualized care plans

Requirements:

* High school diploma or equivalent
* Completion of a Certified Nursing Assistant or Certified Home Health Aide program within the last 24 months
* Valid driver's license, auto insurance, and reliable transportation
* At least one year of experience as a Home Health Aide or Certified Nursing Assistant in a healthcare setting
