About the RoleWe're looking for CNAs who are passionate about delivering high-quality, patient-centered care in the comfort of our patients' homes. As a Home Health CNA, you'll work closely with our interdisciplinary team to develop and implement personalized care plans that meet the unique needs of each patient.
Responsibilities- Provide direct patient care, including bathing, toileting, dressing, grooming, and hygiene
- Assist with ambulation, transfers, and range of motion exercises
- Monitor vital signs, intake/output, and document any changes
- Work collaboratively with our team of home health nurses and aides to ensure seamless care delivery
- Communicate effectively with patients, families, and healthcare team members
- High school diploma or equivalent
- Active Nursing Assistant Certification in the state of Colorado
- BLS/CPR certification
- Compassionate nature, good communication skills, and ability to lift up to 50 lbs
- Pass a background check and drug test
