Job Title: Home Health LPN Specialist

About the Role:

This role is a crucial part of our interdisciplinary team, delivering high-quality patient care in a home setting. The successful candidate will model Compassus values and promote our philosophy while upholding the Code of Ethical Conduct and fostering positive working relationships.

Key Responsibilities:

  • We implement and follow nursing care plans for each patient to ensure effective treatment and recovery.
  • The Home Health LPN provides skilled nursing services, treatments, and diagnostic procedures as assigned by the physician and Registered Nurse Case Manager (RNCM).
  • We initiate preventive and rehabilitative nursing procedures as necessary to promote patient well-being.
  • The LPN observes signs and symptoms, reporting reactions to treatments and changes in the patient's physical or emotional condition to the physician and RNCM.
  • We teach and counsel patients and their families/significant others regarding nursing care needs and related problems.
  • The LPN evaluates the effectiveness of LP/VN services with the RNCM, under their guidance.
  • Maintain accurate and complete records of observations, treatments, and patient care.
  • Participate in medical record audits and staff meetings as scheduled.
  • Submit schedule changes to the Director of Clinical Services/Clinical Supervisor daily.
  • Prepare clinical and progress notes, submitting them within 24 hours of visit.
  • Assist physicians and RNCMs in performing specialized procedures.
  • Prepare equipment and materials for treatments.
  • Observe aseptic techniques as required.
  • Assist in patient learning and self-care techniques.
  • Ensure all care is provided with respect for patient rights.
  • Report grievances and complaints to the appropriate persons.
  • Follow standard precautions and infection control procedures.

Requirements:

  • Certificate from an accredited program of Practical or Vocational Nursing, approved by the state.
  • Two years of experience as an LPN in a clinical care setting preferred.
  • Experience in skilled nursing, preferably in hospice, home health care, oncology, or emergency care preferred.

Education and Certification:

  • Active and unencumbered Licensed Practical Nurse or Licensed Vocational Nurse in the state(s) of employment required.

Salary Information:

The estimated annual salary for this position is approximately $77,500, based on location and experience.
