Angel Care Home Health is a small, family-oriented agency dedicated to providing exceptional patient care. We are passionate about making a positive impact on the lives of our patients and their families.
As a physical therapist with our agency, you will have the opportunity to work with a diverse range of patients and develop your clinical skills in a dynamic and supportive environment.
**Key Responsibilities:**
- Provides physical therapy services to patients according to a written physician's plan of care.
- Instructs other personnel and/or family/caregiver members in certain phases of physical therapy.
- Identifies patient and family/caregiver needs for other home health services and refers as necessary.
- Provides physical therapy consultation to home families/caregivers when indicated.
**Requirements and Qualifications:
To be successful in this role, you will need to possess:
- Licensed Physical Therapist (PT)
- Newly graduated physical therapists are welcome.
- Preferred: 2 years of physical therapy experience or a combination of experience and education.
- Experience preferred: 1 year of home health experience.
- Current CPR card for Healthcare Providers.
- Must be a licensed driver with an automobile that is insured and in accordance with state requirements.
- Ability to travel to see patients in their homes.
- Demonstrates good verbal and written communication, and organization skills.
