At BayCare HomeCare, we are committed to delivering exceptional patient care in the comfort of their own homes. As a Per Visit Home Health LPN, you will play a vital role in providing hands-on care to our patients, utilizing the nursing process and clinical standards to ensure the highest level of care.

Responsibilities:
  • Deliver patient care to assigned patients, providing care as ordered, including IV infusions, infusion pumps, and tube feedings under the supervision of a Registered Nurse.
  • Identify, implement, and maintain appropriate teaching plans for patients and caregivers under the direction of a Registered Nurse.
  • Follow the plan of care and collaborate with the Registered Nurse regarding patient response to treatment and change in status.
  • Utilize nursing knowledge in the administration of medication and evaluation of effectiveness when applicable.
Requirements:
  • BLS (Basic Life Support) certification.
  • LPN (Registered Nurse) license.
  • Driver's License, State of Florida (Florida driver's license is required within 30 days of hire).
  • Required Associate's or Diploma in Nursing.
  • Required 1 year Licensed Practical Nurse experience.
  • Preferred 1 year Home Care experience.
