Position Overview: The Registered Nurse Case Manager is responsible for delivering expert nursing care to patients facing terminal illnesses, guided by a continuous assessment of their needs. This role involves the planning, execution, and evaluation of nursing interventions through a systematic evaluation of health requirements and optimal resource utilization.

Key Responsibilities:

  • Utilize clinical assessment tools and data to evaluate patient requirements.
  • Develop personalized and comprehensive case management and treatment strategies.
  • Provide education to patients and their caregivers regarding available benefits, services, and resources.
  • Document intricate medical information to enhance patient outcomes.
  • Adhere to case and quality management protocols in alignment with regulatory standards and organizational policies.
  • Exhibit strong written and verbal communication skills with patients, healthcare providers, and other stakeholders.
  • Demonstrate the ability to assess and respond to the needs of patients and families promptly.
  • Possess a foundational understanding of case management practices, including knowledge of relevant products and services.
  • Maintain basic computer proficiency, particularly with the Microsoft Office suite.
  • Establish and nurture professional and collaborative relationships with patients, families, and healthcare providers.
  • Provide care and support to patients and families during scheduled on-call hours.
  • Work hours are Monday to Friday, from 8:00 am to 4:30 pm, with a weekend rotation as required.

Employment Status: Exempt with benefits.

Qualifications:

  • Professional: Must be a graduate of an accredited School of Nursing and hold current licensure or eligibility for licensure in Kentucky and Ohio. A minimum of one year of experience in a patient care role within a hospital, nursing home, or similar healthcare setting is preferred.
  • Personal Attributes: Must possess qualities of empathy, accountability, and reliability. Effective communication skills with team members, patients, providers, and family members are essential. Ability to work independently without direct supervision is required. Demonstrated leadership and teaching capabilities are also necessary.
  • Physical Requirements: Must be able to travel independently to client homes, nursing facilities, hospitals, and other locations. Proper use of personal protective equipment and adherence to universal precautions is mandatory. The ability to lift a minimum of 50 pounds using appropriate body mechanics is required.
