Job Summary

Bridge Home Health and Hospice is seeking a skilled Speech Therapist to join our team. As a Speech Therapist, you will be responsible for implementing standards of care for speech-language pathology services and adhering to all conditions in the Service Agreement.

Key Responsibilities
  • Evaluation and Assessment: Conduct thorough evaluations and assessments of patients to identify functional speech defects and establish a plan of care to improve their function.
  • Speech Therapy Program Administration: Administer speech therapy programs utilizing specialized therapeutic techniques and equipment.
  • Communication Device Recommendations: Recommend communication devices and aids as indicated.
  • Collaboration and Coordination: Confer with referring physicians and other organization personnel to ensure coordinated and comprehensive care contributes to the total plan of care.
  • Documentation and Record-Keeping: Prepare and submit notes per organization policy and maintain clinical competency in speech-language pathology practice and theory.
  • Patient and Family Education: Instruct patients, family members, and caregivers in the patient's treatment regime as indicated.
  • Supervision and Leadership: May supervise home health aides in procedures directly related to the speech-language therapy plan of care.
Qualifications
  • Education and Licensure: Possess a master's or doctoral degree in speech-language pathology (SLP) and meet either of the following requirements:
    • Be licensed as a speech-language pathologist by the State in which the individual furnishes such service.
    • If the State does not license speech-language pathologists:
    • Successfully complete 350 clock hours of supervised clinical practicum (or be in the process of accumulating such supervised clinical experience).
    • Performed not less than 9 months of supervised full-time SLP services after obtaining a master's or doctoral degree in SLP or a related field.
    • Successfully completed an approved national examination in SLP.
  • Experience: A minimum of one (1) year's clinical experience in Speech-Language Pathology.
  • Skills and Certifications: Demonstrate good verbal and written communication, and organization skills. Possess and maintain current CPR certification. Be a licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
