Position Overview: East Bay Community Action Program (EBCAP) is looking for a dedicated full-time Registered Nurse Care Manager to join our Behavioral Health Services Division.

Role Responsibilities: The ideal candidate will be responsible for conducting comprehensive health evaluations and managing care for adult clients. This includes:

  • Assisting with patient advocacy and ensuring access to community health resources.
  • Providing referrals to relevant community services and agencies.

Qualifications: Candidates must possess at least an Associate's Degree in Nursing and hold a valid Registered Nurse (RN) license from the appropriate health department.
