Position Overview: East Bay Community Action Program (EBCAP) is looking for a dedicated full-time Registered Nurse Care Manager to join our Behavioral Health Services Division.
Role Responsibilities: The ideal candidate will be responsible for conducting comprehensive health evaluations and managing care for adult clients. This includes:
- Assisting with patient advocacy and ensuring access to community health resources.
- Providing referrals to relevant community services and agencies.
- Collaborating with both internal and external healthcare providers to facilitate clients' access to necessary services.
Qualifications: Candidates must possess at least an Associate's Degree in Nursing and hold a valid Registered Nurse (RN) license from the appropriate health department. Experience in Psychiatric Nursing is preferred, as is familiarity with electronic medical records (EMR), although training will be provided for EBCAP's specific EMR system.
