Key Responsibilities
  • Population Health Management: Oversee population health management initiatives, including risk stratification, care coordination, and patient engagement strategies to improve health outcomes and reduce healthcare disparities.
  • Clinical Guidelines and Protocols: Develop and implement clinical guidelines, protocols, and best practices to standardize care delivery and promote evidence-based medicine.
  • Value-Based Care Strategies: Collaborate with executive leadership, clinical teams, and external stakeholders to develop and implement value-based care strategies that improve patient outcomes, reduce costs, and enhance the overall quality of care.
  • Clinical Leadership: Treat patients as the direct provider in collaboration with advanced practice providers and interdisciplinary teams, providing evidence-based care including complex care through pharmacological and non-pharmacological interventions.
  • Leadership and Management: Lead the development and implementation of clinical initiatives, and provide guidance and support to clinical teams to ensure successful execution.
Requirements
  • Medical Degree: Medical degree from an accredited medical school.
  • Board Certification: Board certification in Internal medicine, geriatrics, or family medicine.
  • Medical License: Active medical license and DEA registration in at least one state and willing to get licensed in multiple states in the region.
  • Clinical Experience: Minimum of 3 years of clinical practice experience, with demonstrated leadership and management experience in healthcare delivery or administration.
  • Analytical and Communication Skills: Excellent analytical skills, with the ability to interpret data, analyze trends, and make data-driven decisions. Exceptional communication and interpersonal skills, with the ability to effectively engage and influence stakeholders at all levels of the organization.
