Mass General Brigham is seeking a skilled Registered Nurse (RN) Case Manager to join our experienced Case Management team. This role focuses on transition planning and Utilization Review, with a goal of assessment and development of initial transition plans.
The ideal candidate will have a high degree of skill in written and oral communications, as well as the ability to work in a fast-paced environment. They will be responsible for initial assessment and development of transition plans, participation in multidisciplinary rounds, utilization review, patient advocacy, and ability to delegate tasks to Case Management assistants.

Responsibilities:
- Initial assessment and development of transition plans
- Participation in multidisciplinary rounds
- Utilization review
- Patient advocacy
- Ability to delegate tasks to Case Management assistants
Requirements:
- High degree of skill in written and oral communications
- Ability to work in a fast-paced environment
- Three or more years of clinical experience in a healthcare delivery system
- Completion of an accredited Nursing program
- Current Nursing License in the State of MA
Preferred:
- Case Management, Home Care Discharge Planning experience
