Job Summary:

UVM Home Health & Hospice is seeking a skilled and dedicated High Tech LPN to provide specialized nursing care in the home and at a local summer camp. The ideal candidate will have a passion for delivering compassionate care and developing meaningful relationships with patients and their families.

Key Responsibilities:

Provide high-tech nursing care to technology-dependent patients in their homes
Collaborate with the healthcare team to ensure comprehensive patient care
Perform assessments, administer medications, and educate patients and families
Utilize innovative technology to monitor and manage complex medical conditions

Requirements:

Current LPN license in Vermont
Two or more years of directly related home/public health, hospice and/or maternal/child health nursing experience
Previous home care experience a plus
Strong clinical skills and a commitment to patient-centered care
Excellent communication, collaboration, and critical thinking skills
