Job Summary

As a Registered Nurse in our Outpatient Dialysis team, you will be responsible for providing high-quality patient care and education to patients with end-stage renal disease. You will work closely with our interdisciplinary team to develop and implement individualized care plans, ensuring that patients receive the best possible outcomes.

Responsibilities

Provide direct patient care, including assessment, planning, intervention, and evaluation of patients with end-stage renal disease.

Develop and implement individualized care plans, taking into account patient preferences, medical history, and treatment goals.

Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.

Educate patients and their families on dialysis treatment options, medication management, and self-care techniques.

Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.

Requirements

Current RN licensure in the state of employment.

Minimum of 9 months of experience as a Registered Nurse, preferably in a nephrology setting.

Chronic/acute hemodialysis experience preferred.
