Job SummaryThe primary functions of this role are to perform admissions and administer nursing care for patients of all ages in their place of residence, coordinate care with the interdisciplinary group, patient/family, and referring agency, and assume the responsibility for coordination of care.
Requirements- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Current state license as a Registered Nurse. Proof of current CPR certification. Current Driver's License.
- Two years' experience as a Registered Nurse in a clinical setting and one-year full-time home health experience preferred but not required.
- Knowledge of OASIS highly preferred but not required.
- Assess home health patients utilizing the Nursing Assessment and LCD criteria to identify physical, psychosocial, and environmental needs.
- Develop and document the plan of care in correlation with the assessment.
- Provide nursing care utilizing infection control measures (OSHA).
- Work with the Interdisciplinary Group to ensure patient/family needs are met.
- Communicate with attending physicians, home health physicians, and other physicians involved in the patient's care.
- Timely submission of all required paperwork.
- Meet productivity standards.
- Perform on-call responsibilities and provide on-call service to patients/families as assigned.
