We are seeking a skilled Registered Nurse to join our team as a Home Hemodialysis RN. As a key member of our healthcare team, you will provide comprehensive patient care in an independent setting, empowering patients to take control of their health.
Responsibilities:- Provide patient education on Home Hemodialysis, enabling patients to administer their own dialysis and take ownership of their health.
- Develop and implement complete care plans, assessing, diagnosing, and tracking patient progress.
- Work independently, building deep relationships with patients, support systems, and care partners.
- Manage your own cases, with flexibility to schedule and prioritize tasks.
- Current driver's license, reliable transportation, and vehicle insurance in state of residence.
- Associates Degree in Nursing (A.D.N) from accredited school of nursing required; Bachelor of Science in Nursing (B.S.N); three-year diploma from accredited diploma program may be substituted for nursing degree.
- Basic computer skills and proficiency in MS Word and Outlook.
