Key Responsibilities- Conduct thorough clinical assessments of patients to identify their needs and develop personalized care plans.
- Collaborate with healthcare professionals to ensure seamless transitions of care and provide exceptional patient outcomes.
- Communicate effectively with patients, families, and healthcare teams to address concerns and provide education on our services.
- Manage multiple priorities simultaneously and maintain accurate documentation to ensure compliance with regulatory requirements.
- Develop strong relationships with healthcare providers, facilities, and community organizations to promote our services and improve patient care.
- Bachelor's degree in nursing or related field.
- Current Florida license as RN, current active single-state out-of-state nurse license, or current active multistate/compact nurse license.
- Minimum of two (2) years of RN experience in acute care, hospice, home health care, or other post-acute care.
- Strong clinical assessment skills and experience working with electronic medical records.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
- Ability to work in a fast-paced environment and manage multiple priorities simultaneously.
- Satisfactorily complete competency requirements for this position.
- Represent the Company professionally at all times through care delivered and/or services provided to all clients.
- Comply with all State, federal and local government regulations, maintaining a strong position against fraud and abuse.
- Comply with Company policies, procedures and standard practices.
- Observe the Company's health, safety and security practices.
- Maintain the confidentiality of patients, families, colleagues and other sensitive situations within the Company.
