At ServiceNet, we're seeking a dedicated Case Manager to join our team. As a Case Manager, you'll play a vital role in helping individuals facing persistent mental health and substance use challenges to live fulfilling lives.

Key Responsibilities:
  • Provide care coordination for clients with mental health and substance use challenges, both in the community and in the office.
  • Coordinate care with mental health and healthcare providers to create a seamless, integrated care experience.
  • Assist clients in navigating services such as legal aid, housing, employment, substance use treatment, medical care, and more.
  • Collaborate with your team to ensure each client receives the support they need to thrive.
  • Build partnerships within the community to make it easier for clients to access the resources they need.
Qualifications:
  • A BA/BS in Social Work or a related field.
  • At least 3 years of experience in care coordination.
  • Someone who is compassionate, an excellent communicator, and ready to make a difference.
  • The ability to travel, with a valid driver's license and reliable vehicle.
