Responsibilities
  • Conduct regular wellness visits for all residents to assess their health and identify any potential issues.

  • Monitor and document resident vital signs, including weight, blood pressure, and temperature, to ensure accurate and timely reporting.
  • Provide education and support to residents and their families on health-related topics, such as medication management and disease prevention.
  • Work closely with the healthcare team to ensure seamless communication and coordination of care.
  • Participate in quality improvement initiatives to enhance the overall quality of care and services provided to residents.
Requirements
  • Graduate of an approved college or school of nursing.
  • Current state license as a professional Registered Nurse (RN).
  • Minimum of one year of experience in a healthcare setting, preferably in a long-term care or assisted living environment.
  • Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare team members.
  • Ability to work independently and as part of a team, with a strong focus on customer service and resident satisfaction.
