- Collaborate with the Resident Care Director to develop and implement individualized care plans that meet the unique needs of each resident.
- Monitor and document resident vital signs, including weight, blood pressure, and temperature, to ensure accurate and timely reporting.
- Provide education and support to residents and their families on health-related topics, such as medication management and disease prevention.
- Work closely with the healthcare team to ensure seamless communication and coordination of care.
- Participate in quality improvement initiatives to enhance the overall quality of care and services provided to residents.
- Graduate of an approved college or school of nursing.
- Current state license as a professional Registered Nurse (RN).
- Minimum of one year of experience in a healthcare setting, preferably in a long-term care or assisted living environment.
- Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare team members.
- Ability to work independently and as part of a team, with a strong focus on customer service and resident satisfaction.
