Key Responsibilities
  • Assessment and Care Planning: Participate in initial assessments, care planning, and ongoing re-assessments of participant care, ensuring that their needs are met and their goals are achieved.
  • Interdisciplinary Team Collaboration: Attend IDT meetings, actively participate in team discussions, and contribute to the development of participant care plans.
  • Assessment and Documentation: Complete all assigned assessments, including initial, semi-annual, annual, service request, and significant event assessments, and maintain accurate and timely documentation in participant medical records.
  • Psychosocial Support: Assess the psychosocial needs of participants and provide supportive counseling, working collaboratively with behavioral health providers to ensure comprehensive care.
  • Admissions and Discharges: Facilitate hospital, rehabilitation, and nursing home admissions and discharges, ensuring that PASRR documentation is completed for NH admission.
  • Conversion Process: Assist in the conversion process of participants from community to long-term care, working collaboratively with Medicaid Specialists, skilled nursing facilities, and participant caregivers to complete the process.
  • Family Meetings and Referrals: Arrange and facilitate family meetings as needed and refer participants and families to appropriate community services, acting as a liaison and/or advocate with community organizations.
  • Professional Development: Maintain professional, accurate, and timely social service documentation in participant medical records and conduct participant council meetings as assigned.
  • Collaboration and Communication: Work collaboratively with the Director of Social Work and Behavioral Health providers to ensure guardianship is up to date, educate participants regarding health care proxy (HCP), and assist participants in completing HCP forms.
  • Insurance Benefits and Fiscal Information: Work collaboratively with the fiscal department to maintain participant insurance benefits and complete required documentation of fiscal information in medical records.
  • Plan of Care Review: Review the plan of care with participants, guardians, and/or activated health care proxies as assigned.
  • Authorizations and Home Visits: Complete authorizations for home care and other approved services timely and accurately and conduct home and/or skilled nursing facility visits to assess participants as indicated.
  • Palliative Care and End-of-Life Planning: Work collaboratively with the Palliative Care team, assist with end-of-life planning as indicated, and provide timely communication to appropriate staff regarding disenrollment, conversion to long-term care, transfer of sites, participant and/or caregiver demographic changes.
  • Reporting and Compliance: Report allegations of abuse to the appropriate state agency, provide support and resources to participants as they will accept, and complete required documentation.
Requirements
  • Education and Licensure: Current Social Work licensure in the Commonwealth of Massachusetts at the Masters level (L.I.C.S.W. or L.C.S.W.) required.
  • Experience: Minimum of 1 year of experience in Social Work providing traditional Clinical or Case Management services with a geriatric population.
  • Certifications: Current C.P.R. Certification or ability to become certified.
  • Skills and Abilities: Treat all participants in a welcoming and professional manner, possess strong verbal, written, and listening skills, and have the ability to multi-task in a fast-paced environment.
