Hospice RN Job Description

We are seeking an experienced Hospice RN to join our team at Della Infotech in Walla Walla, WA. The ideal candidate will have at least 2 years of Hospice case management experience and be licensed in Washington and Oregon states.

Key Responsibilities:

  • Manage a case load of 15 patients with 4-5 visits per day
  • Complete charting same day
  • Perform all service level nursing, including routine, GIP, continuous, and respite care
  • Admission, SIA, and death visits

Requirements:

  • 2 years of Hospice case management experience required
  • Acute/Med Surg experience preferred
  • BLS (AHA) certification required
  • Active DL and auto insurance required
