About the Job

We are seeking a dedicated Wound Care Registered Nurse to join our team at Providence Home and Community Care in Snohomish County, Washington.

Job Summary:

This full-time position offers a unique opportunity to work with patients in their homes, providing whole-person care and improving their quality of life. As a Wound Care RN, you will be part of an interdisciplinary team that works together to deliver comprehensive care.

Responsibilities:

  • Provide high-quality, patient-centered care in a home-based setting
  • Collaborate with healthcare teams to develop and implement individualized care plans
  • Conduct wound assessments and provide education to patients and families on wound management
  • Work closely with other healthcare professionals to ensure seamless care transitions
  • Document patient progress and communicate effectively with the care team

Requirements:

  • Bachelor's Degree in Nursing or equivalent experience
  • Washington State Registered Nurse License
  • National Provider BLS certification within 30 days of hire
  • National Provider Identifier (NPI) number within 30 days of hire
  • 2 years of experience as a registered nurse in a Long Term Care, Medical/Surgical Unit, or Skilled Nursing Facility
  • 1 year of recent hands-on technical clinical experience in a Medicare-certified Homecare or Hospice agency/facility
