- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
- Maintain accurate and detailed records of patient interactions, including progress notes, medication administration records, and other relevant documentation.
- Current RN licensure in the State of New Jersey.
- At least 1 year of experience in hospice, home care, or community health.
- Ability to work independently with minimal supervision and as part of a team.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Strong problem-solving and critical thinking skills, with the ability to prioritize tasks and manage multiple responsibilities.
