Job Summary:

The Home Care LPN provides direct nursing care as prescribed by physicians to acutely ill and/or chronically ill patients in their homes. Responsibilities include recording clinical and progress notes, coordinating patient care activities, and working under the supervision of a Registered Nurse or designated supervisor.

Responsibilities:

  • Provides high-quality clinical services within the scope of practice, following state laws and agency policies.
  • Documents patient visits thoroughly and accurately in the electronic medical record.
  • Participates in the agency's care coordination process, including timely reporting of patient needs and changes in condition.
  • Maintains confidentiality of patient health information.

Requirements:

To be successful in this role, you must have current LPN licensure in the state of practice, a valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation. Previous home care experience is preferred, but not required.
