About the Role

Aveanna Healthcare is seeking a dedicated and compassionate Private Duty Nurse LVN/LPN to join our team. As a Private Duty Nurse, you will play a vital role in providing one-on-one care to our patients, helping them transition from hospital to home and ensuring they receive the continuous care they need to thrive.

Key Responsibilities
  • Implement physicians' prescribed plans of care, ensuring seamless coordination and delivery of services.
  • Conduct patient assessments and coordinate care with healthcare teams to address patients' unique needs.
  • Administer prescribed medication, treatments, and therapies, prioritizing patient safety and well-being.
  • Document care provided in a timely and compliant manner, maintaining accurate records for patient and team reference.
  • Teach and train family members on health promotion, disease management, and medication administration, empowering them to support their loved ones.
  • Communicate effectively with physicians, clients, family members, staff, and Case Managers to address patient concerns, changes in condition, or other issues.
Requirements
  • Active, unencumbered license (RN/LPN/LVN) in the state of practice.
  • Current CPR certification from American Red Cross, American Heart Association, or BLS.
  • TB skin test (current within the last 12 months).
  • At least six months of prior hands-on nursing experience (preferred but not required).
  • Reliable transportation for patient visits.
