We are seeking a skilled RN Case Manager to join our team at Providence. As a key member of our care team, you will be responsible for coordinating the ongoing case management of an assigned panel of patients.
Key Responsibilities- Coordinate with a multidisciplinary team to provide high-quality care to patients.
- Assume responsibility for the ongoing case management of an assigned panel of patients.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with patients, families, physicians, and other healthcare professionals to ensure seamless care coordination.
- Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
- Graduation from an accredited school of nursing.
- Alaska Registered Nurse License upon hire.
- 2 years of Professional nursing experience.
- Bachelor's Degree in Nursing.
- 2 years of Case Management in a clinic or outpatient setting.
