Neuro Rehab Social Worker

As a Neuro Rehab Licensed Master Social Worker, you will play a vital role in our team's efforts to provide comprehensive care to our patients. Your expertise and compassion will help us make a meaningful difference in the lives of those we serve.

Key Responsibilities:

• Develop and implement social work care plans to support patients' treatment episodes.
• Collaborate with multidisciplinary teams to ensure timely discharge planning and effective communication with external stakeholders.
• Advocate for patients' rights and benefits, serving as a liaison between patients, families, and healthcare providers.
• Facilitate real-time communication and coordination with external Nurse Case Managers, insurance adjustors, and physicians.

Requirements:

• Master's degree in Social Work.
• Three years of case management experience or related social work experience and full license (LMSW).
