About the Position
Adelante Healthcare is seeking a skilled Registered Nurse to join our team. In this dynamic role, you will have the opportunity to make a positive impact on patients' lives by providing high-quality care and education.

Job Responsibilities
As a Registered Nurse at Adelante Healthcare, your primary duties will include:
1. Performing triage calls using approved clinical decision support tools for adult primary care, OB, and Peds populations.
2. Participating in team meetings, quality and process improvement initiatives, and designated Specialty Team meetings.
3. Facilitating health and disease patient education sessions for adults, pediatrics, and OB populations.
4. Coordinating continuity of patient care with external healthcare organizations and facilities, including referrals to outside specialties and ongoing communication between providers and care teams.
5. Developing relationships with external healthcare organizations and facilities to find additional resources for patients.
6. Coordinating outreach efforts to high-risk patients and hospital discharge and admissions, collaborating with health plans, hospitals, and skilled nursing facilities for transitional needs.
7. Conducting comprehensive, preventative screenings for patients and assisting support staff in daily patient interactions as needed.
8. Promoting clear communication amongst the care team and treating clinicians by ensuring awareness regarding patient care plans.
9. Facilitating patient care based upon approved standing orders and protocols.
10. Working collaboratively with the clinical team on data collection, health outcomes reporting, clinical audits, and programmatic evaluation.

Requirements
To be successful in this role, you must have:
1. Graduation from an accredited nursing program.
2. Current licensure with the AZ State Board of Nursing in good standing.
3. Demonstrated understanding of clinical methods and techniques used in record-keeping systems.
4. Ability to implement the principles and practices of supervision.
5. Advanced computer efficiency in Microsoft Office suite (Word, Excel, PowerPoint, Outlook) and experience in Patient Management System.
6. Meet organization immunization requirements.
7. Valid Arizona driver's license.
8. Two to five years of experience in a clinical setting.
9. Certification to perform cardiopulmonary Resuscitation for the Health Care Professional (CPR) and AED through courses following American Heart Association and Red Cross guidelines.
10. Valid Level One Fingerprint Clearance Card issued by the Arizona Department of Public Safety for all specialty behavioral health locations.

Additional Skills
Prior experience in electronic health records and bilingual skills (Spanish/English) are highly valued but not required.
