Key Responsibilities:
• Assess patients with physical, cognitive, and psychosocial challenges to determine the suitability of physical therapy interventions.
• Develop, implement, and review patient care plans in collaboration with physicians and the healthcare team to enhance patient well-being.
• Maintain comprehensive documentation of care provided and actively participate in care coordination with other healthcare professionals.
• Educate and train family members and caregivers on patient care techniques.
• Supervise Certified Occupational Therapist Assistants and Aides, ensuring care aligns with patient needs.

Qualifications:
• Valid State License as a Physical Therapist.
• Current driver's license and certification in CPR.
• Preferred: Minimum one year of experience as a Physical Therapist.
