Key Responsibilities:

  • Evaluate clients' needs and consult with the treatment team to develop effective treatment plans.
  • Facilitate individual and family sessions, therapeutic groups, and maintain accurate clinical records.
  • Provide documentation and support for utilization review, ensuring seamless care coordination.
  • Participate in aftercare planning and outcome data process to measure treatment effectiveness.
  • Conduct Level of Care Assessments as needed to ensure clients receive the appropriate level of care.
  • Other duties as assigned to support the team and enhance client care.

Requirements:
