About the Role

This is a 13-week contract position, working 40 hours per week, with an estimated salary of $85,120 (based on $2,040 per week). You will be responsible for providing physical therapy services to patients in their homes, working closely with healthcare professionals to develop and implement individualized treatment plans.

Key Responsibilities
  • Provide physical therapy services to patients in their homes, including assessments, treatments, and progress monitoring.
  • Collaborate with healthcare professionals to develop and implement individualized treatment plans.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care delivery.
  • Maintain accurate and timely records of patient interactions and treatment plans.
Requirements

To be successful in this role, you will need:

  • An active unrestricted Oregon license as a Physical Therapist Assistant.
  • A current BLS certification from the American Heart Association.
  • At least 1 year of skilled home health experience.
  • The ability to provide at least 1 supervisory reference.
  • The primary series COVID vaccination and one booster.
